Proctor Forms (These forms are for graduate students only) Course Exam Proctor Form (Undergraduate students should complete the Undergraduate Proctor Form) *Comprehensive Exam Proctor Form *Effective Term 3, 2008 eCampus students who have to retake the Comprehensive Exam will be charged an additional fee of $50. The fee will apply each time the exam is taken.
