If a department or honor society wishes to have a new award approved for Honors Convocation the application must be submitted by the second Monday in January for approval by Honors Convocation committee. The following information must be submitted in a word document. Please carefully review the Rules and Procedures for Individual Awards before preparing this document. - Name, email and extension of individual submitting the application
- Proposed name of award
- Criteria for selection of award
- Significance of award
